About Foundational Concepts

Founded in 2013 by physical therapists Jennifer Cumming and Sarah Dominguez, Foundational Concepts was born out of a shared passion for pelvic health and a desire to create meaningful change in the field of physical therapy. With over two decades of combined experience in pelvic floor physical therapy, both Jenn and Sarah had worked in various capacities and settings since 2001. Through this experience, they noticed a glaring gap: a lack of awareness and specialized care for patients dealing with pelvic dysfunction.

Recognizing the need for a safe, welcoming space where patients could receive specialized, one-on-one care, Jenn and Sarah knew they could make a difference. They envisioned a practice where patients could have an hour of undivided attention, allowing therapists to listen deeply and fully assess each individual’s unique needs.

After extensive brainstorming and research, Jenn and Sarah took the brave step to open their own specialty practice in physical therapy. Their mission was clear:

“Foundational Concepts, Specialty Physical Therapy will provide comprehensive, evidence-based specialty physical therapy services to both men and women in a healing environment, restoring function by treating the whole person. We are committed to educating physical therapy students to understand the whole person approach to therapy, and sharing our expertise with other healthcare providers to ensure that pelvic dysfunction is addressed in a timely manner.”

Since its founding, Foundational Concepts has grown to serve the Kansas City area and beyond. The practice now operates in four locations, with a fifth location set to open in Lawrence, Kansas, in May of 2025. At each step of the way, Jenn and Sarah have maintained a hands-on approach to mentoring staff, ensuring the highest quality of care and keeping abreast of the most current, evidence-based practices in pelvic health.

Both Jenn and Sarah are Board Certified in Women’s Health Physical Therapy by the American Board of Physical Therapy Specialties. They are deeply committed to elevating the field of pelvic floor physical therapy, not only by educating other physical therapists but also by providing free community education. Through online and in-person events, they empower both men and women to take control of their pelvic health and seek the help they need.

This journey has allowed Foundational Concepts to create a model of care that benefits both patients and staff. Jenn and Sarah are proud to have built a practice that not only restores health and function for their patients but also supports a healthier work-life balance for their employees. From the beginning, they’ve strived to align their personal values with their professional goals, creating a fulfilling and sustainable model of care.

Advocating for women in healthcare remains at the heart of their work, especially as burnout continues to be a widespread issue among healthcare providers. Jenn and Sarah are passionate about the profession and are dedicated to remaining innovative, mindful of growth, and supportive of those who contribute to the success of their practice.

Jennifer Cumming

Jennifer completed her master’s degree in physical therapy in 2001 at Des Moines University, a well-regarded osteopathic based physical therapy program. There she developed excellent orthopedic and manual skills, including muscle energy technique and myofascial release.

Sarah Dominguez

Sarah studied at the University of Missouri in Columbia, Missouri, receiving a Bachelor of Science in Psychology (1998) and a Master of Science in Physical Therapy (2001). She began her training in pelvic floor rehab while at RIC in 2002, along with developing strong manual therapy and an osteopathic approach to musculoskeletal pain and dysfunction.
